- The patient calls the
medical office and describes their complaint/reason for appointment/visit.
- The Physician or other
medical staff member logs into PrimeCare on the Web™.
- The medical person enters
the patient's identifying information (NOT name), age and gender and
selects the appropriate medical history questionnaire and language
(English or Spanish).
- A unique login ID and
password are generated for the patient, and the office gives this
information to the patient.
- The patient goes to the
PrimeCare on the Web™ Patient Site and enters the login ID and password
given to them.
- The selected questionnaire
appears.
- The patient answers the
questions on the web browser, taking their time. The patient can leave
the questionnaire and come back to it as desired. At anytime while
the patient is answering questions, the physician's office can check
the status of the patient's progress.
- When the patient has finished,
the information is processed and saved by PrimeCare on the Web™. The
patient's medical history and diagnostic possibilities report is sent
by fax or electronic mail to the physician's office.
- The physician can also
print or review the report directly from PrimeCare on the Web™.
- When the patient arrives
at the physician's office to complete their appointment, the physician
already has a copy of their report, ready to review with the patient.
